This one came highly recommended from another user and I am happy that I gave it a try. Functionality and usability are rather seamless and I will definitely recommend this to anyone looking for a good forms solution.

Works rather beautifully, providing great functionality without ever getting in way. I liked the clean interface that allows to focus on usability rather than having to sort things out.

Many things to like but frustrating list view with no date only entry number. Can't locate all notifications or emails in email client so unsure of consistency and functionality over time. Can't export list or even view as full on screen. Have to click each one to view full email address. Still playing with it but frustrated with support and easy location of things on website. Has nice clean look to admin side though.

Form was working great, stopped working, stopped getting emails, settings drop down became unavailable, submission details incomplete. Submitted support request, sent email, no response. Impacting my business so now I'll need to find another form pluggin & rebuild.

Needs an X on the popup window to close for no data entered cases. You can close by clicking off the form, but not intuitive. Also, sends the person contacting a reply so why not auto send an email to the admin with the message details at the same time? Would be nice to be able to select that the email to the admin or selected email address also contained the attachments, if any. Otherwise decent enough.

Its easy to manage and use. Best solution for making customized contact form.
